2014 Summer Camp Registration

Steps to completing camp registration:

  1. Register Online
    Click on the "Register Now" button below to complete the online registration. At the end of the online registration process you will have the option to pay with a credit card, send in a check, or submit a scholarship request.
     
  2. Submit Supplemental Registration Packet
    • Download, Read and Complete the Supplemental Registration Packet (PDF).
      (To fill out the packet using your computer, please use Adobe Reader).
             
    • The packet include the following forms:
      - Medical Release Form (Requires Neurologist's signature!)
      - Agreement Form
      - Release & Consent Form
      - Release & Indemnity Form
      - Scholarship Application Form (if you’re applying for a full or partial scholarship)
                   
    • Return the completed and signed Packet by June 30, 2014, via email, fax, or mail:
      - Email a scanned copy of the packet to marcee@epilepsycolorado.org
      - Fax a copy to 303.377.0081
      - Mail packet to Epilepsy Foundation of CO, 5889 Greenwood Plaza Blvd, Ste 404, Greenwood Village, CO 80111
             
  3. Download and carefully read the Parent Information Packet which includes necessary information about camp.
